GMS BAND AND CHOIR
SIX FLAGS TRIP
1. All Honor Band, Jazz Band, and Red Devil Concert Choir members are eligible to make the Six Flags trip. Members of the Sixth Grade Concert Band, colorguard, and banner carriers will also be eligible to make the trip on a first come, first serve basis. The cost of the trip is $315.00 per student. All money, permission forms, medical forms, and t - shirt order forms for the trip are due no later than Friday, March 14th.
2. All Honor Band members, Jazz Band members and Red Devil Concert Choir members to be able to particpate in the trip must:
Attend all Honor Band, Jazz Band and Red Devil Choir rehearsals
Play or sing their music satisfactorily for Mr. Lane or Ms. Ward
Display proper behavior at all times
Attend a mandatory parent / student meeting for the trip
Parents will be contacted if a member is in jeopardy of being
removed from the trip.
3. All band and choir members will be assigned a chaperone and must stay with their chaperone at all times during the trip. Chaperones must pay a $50 non-refundable fee to be eligible to make the trip. We will stay at the Comfort Inn Suites - Northlake in Atlanta on Friday and Saturday nights, April 25th and 26th. Chaperones will room two to a room next door to the student’s rooms, where there will be three to four students to a room..
4. All students and chaperones must wear their Mighty Red Devil band or choir t - shirt at all times during the trip, including all three days. A t - shirt order form can be found at the end of this letter, or give Mrs. Christie a call at 754 - 6051 to order one.
5. We will be taking 4 buses chartered by Crown Coach in Memphis. Roll will be called each time students re-enter the buses.
6. Parents and students must attend a mandatory parent and student meeting for the trip which will be Tuesday afternoon, April 22nd at 4:15 PM or 6:30 PM in the GMS Cafeteria. If parents and students are not present for the meeting, your student will not be permitted to participate in the trip. Chaperones must attend a mandatory chaperone meeting following the parent and student meeting.
7. The complete schedule for the trip can be found at the end of this letter.
8. The use of alcohol, tobacco, the possession of illegal drugs, and / or shoplifting are not permitted any time during the trip. The breaking of these rules will result in automatic dismissal from any other band or choir school sponsored activity and you will be turned over to Mr. Joy for further consequences according to Shelby County School rules. This is a school field trip. Mr. Lane and Ms. Ward are responsible for all of the students on this trip. The use of alcoholic beverages will not be permitted by the adults and adults should go elsewhere to smoke.
9. Appropriate school dress code must be maintained at all times. School dress code rules will apply for the entirety of the trip, and the red band and choir t - shirts must be worn at all times.
10. All money, permission forms, and medical forms for the trip will be due no later than Friday, March 14th along with the $315 trip fee. There will be NO refunds due to the fact that the money has been used to pay for the buses and the entry fees. Chaperone fees of $50 each are also due no later than Friday, March 14th.
11. Parents must turn in a complete medical form for their child no later than Friday, March 14th or your student will not be permitted to attend. All medicines that need to be administered to your child during the trip must be given to your child’s chaperone on the morning of the trip. Medicines must be in a prescription bottle and placed in a plastic baggie with instructions about how often the medications should be given to your child. Chaperones will administer the medicines to your child. The medical form and permission form are included at the end of this letter. Please fill out both forms in their entirety.
12. Parents checking out their children to keep them in Atlanta should do so with Mr. Lane only. The time to check out your child will be after we have reloaded the buses at 7:00 PM on Saturday evening, April 26th. Mr. Lane will have you sign a release for your child.

I understand and agree Shelby County Board of Education, Germantown Middle School, its band director and chaperones shall not be nor later become liable or responsible in any way in conjuction with services, for any death, damage, delay or irregularity which may occur while participating in this school sponsored activity. I trust the judgement of the Director/Chaperones in regards to my child’s compliance with the trip schedule, regulations, and rules. I further agree to abide by the Director’s decision to terminate my child’s trip because of his or her failure to comply with either the schedule or the regulations and rules.
Also, in case of emergency, I hereby give my consent for a qualified licensed physician to perform any medical or surgical procedures she/he deems necessary to the welfare of my student while participating in this school sponsored activity. I hereby give permission to the trip chaperones to administer prescription medications and Tylenol during the trip. It is understood that the band director/chaperones and the medical personnel will make every attempt to contact parents, guardians, or emergency contacts prior to taking emergency medical actions. Further, this authorization permits said physician to hospitalize, secure appropriate consultations, order injections, blood transfusions, anesthesia (local, general or both) or surgery for this student if such emergency conditions warrant. The undersigned accepts full financial responsibility for all medical services rendered.
This authorization permit shall be effective on April 25th, April 26th, and April 27th, 2008.

Note: All parents with children taking prescription medications are required to turn their child’s medications into their chaperone on the morning of the trip. Medications must be in the prescription bottle placed inside a baggie with instructions included about how often the medication should be administered.
